Payment for services is due at the time of treatment unless alternative arrangements have been agreed in advance. We accept payment by the methods communicated at the practice. Medical aid claims will be submitted on your behalf where applicable, but you remain responsible for any amounts not covered by your medical aid plan.
Unpaid accounts may be referred to a debt collection agency or legal proceedings may be initiated for the recovery of outstanding amounts. All costs associated with debt recovery will be for the account of the patient.
5. Patient Responsibilities
As a patient of the Practice, you agree to:
- Provide accurate, complete and up-to-date medical and personal information at all times.
- Disclose any changes to your health, medications or circumstances that may affect your treatment.
- Follow all pre- and post-treatment instructions provided by Dr Chand or her team.
- Attend follow-up appointments as recommended.
- Treat all practice staff with courtesy and respect.
Failure to comply with these responsibilities may affect the quality of your treatment and the Practice reserves the right to decline or discontinue treatment where appropriate.
